Although this requirement was instituted by Medicare, it has been adopted by many State Boards and other insurance programs. The requirement is that on the initial visit and at each re-evaluation examination, the expected next examination date must be entered as part of the Treatment Plan. Note that for Medicare purposes the patient under Active Treatment must be re-examined at least once every 30 days, and the re-examination must include the use of a Functional Outcome Assessment form. The planned Re-Exam Date can either be typed in or entered by double clicking on the Re-Exam Date field to open the calendar. Once the calendar is open, click on the desired date.
